• Enter provider into credentialing database; load provider into CAQH database if necessary; complete and submit necessary credentialing applications for a wide array of commercial payers, as well as Medicare and Medicaid
• Follow-up on pending applications as needed; load all assigned provider numbers into Horizon Practice Plus™, if applicable
• Maintain provider’s enrollment for all required contracts through periodic re-credentialing
• Maintain up-to-date licensure information files on providers
